Hybrid PET/MRI findings in a case of pulmonary and extra-pulmonary tuberculosis associated with motor-neuron disease-like illness—a coincidence or a causation?: a case report

Abstract Background The association of tuberculosis and motor neuron disease-like illness has not been described previously. We present a case co-existent pulmonary extra-pulmonary in young man whose clinical presentation was suggestive subsequently diagnosed with tubercular infection. This provokes our thought as to whether the between dysfunction just co-incidence, given high prevalence set-up, or does it point towards possible causative role infection disease. Case A 31-year-old presented progressive thinning bilateral upper lower limbs associated pain twitching sensation limb muscles. He had history loss appetite unintentional weight loss. On examination, there evidence fasciculations quadriceps, biceps, paraspinal muscles which further confirmed electrophysiology. work-up for underlying autoimmune, toxic metabolic aetiology, paraneoplastic aetiology found be negative. CT scan chest consolidations lobes multiple tree-in-bud nodules both lungs. Hybrid 18-Flourine-flourodeoxyglucose positron emission tomography magnetic resonance imaging (F-18-FDG PET/MRI) also tuberculosis. Imaging brain revealed atrophy along cortices reduced tracer uptake. Diagnosis nucleic acid amplification test patient put on anti-tubercular therapy. follow-up after 6 months, reported improvement symptoms muscle power limbs. Conclusion have very rare neuron-like illness. It may debated that such an co-incidental; however, signs while therapy, relationship dysfunction. Further epidemiological studies should sought order reach conclusive answer.
